If you decide to extend the residence permit, you need to make a request 60 days before the expiration date (if it was for a year) or 30 days (if it was issued for more than a year).

It happens that you missed the deadline, then you will need to prove to the Italian authorities the seriousness of the reasons why this happened. But this must be done within 60 days from the date of expiry of the residence permit and from what the situation was.
